The purpose of this strand is to provide additional targeted support to children and young people with disabilities and their families to ensure they are able to maximise their life chances.
TRANSITION PROJECT
SNAP Cymru Family and Young People’s officers will provide support for young people with a disability in areas of transition between the ages of 14 – 26.

This project is designed to help and support children and young people and their families address any areas of Transition between the ages of 6 – 14 in Cardiff.
EARLY YEARS PROJECT
This project is designed to help and support babies, children, mums, dads, partners and their families in Cardiff, to give children the best start in life.
EMOTIONAL & MENTAL HEALTH AND WELLBEING
SCHOOLS EXCLUSION FAMILY SUPPORT PROJECT
The School Exclusion Family Support Project will provide accurate and neutral information to families with excluded Children and Young People or those at risk of exclusion. The project will support families’ emotional wellbeing and promote families engagement with the local authority, schools and relevant agencies.
How to refer
Young people and families can self-refer; referrals can also be made by partners in the voluntary and statutory sector (with consent from the young person / family to make the referral).
